NHS 3-Month Free Pass

1. Scan the QR code and submit the contact form to request activation of your membership.
2. Membership can be used at all Everybody Health & Leisure venues except
Everybody@AlderleyPark.
3. Membership only valid to eligible Mid-Cheshire Trust employees as signified by the employer. Proof of employment may be required before leisure centre access is granted.
4. Memberships cannot be exchanged for cash or other goods and services.
5. Standard Everybody Terms and Conditions apply which can be viewed online or requested
at an Everybody leisure centre.
6. Limited availability allocated on a first come, first serve basis, management reserve the right of refusal.
7. Memberships are eligible for 3 consecutive months only and must be activated before the
end of 2024, to expire no later than 31/03/2025.

Internal Use: If presented at venue, assist the customer by scanning the QR code and
submitting the contact form on the website. The customer experience team will contact
the customer regarding their membership application. This membership cannot be activated
in centres, only online.
