Everybody Health and Leisure Live Well on Weight Loss Medication Exercise and Nutrition Programme Terms & Conditions
This programme is exclusively for individuals who are not current Everybody Health and Leisure members, are aged 18 or older, have a BMI of 30 or above, and have been prescribed weight loss medication by their GP or other healthcare licenced provider.
To be eligible you must not have an active membership with Everybody Health & Leisure during the previous 12 months to qualify.
When you register, you agree to participate in the full 12-week programme, attend bi-weekly nutrition and exercise appointments with our Specialist Health Advisors and registered Nutritionist, maintain contact throughout, and complete all required documentation and lifestyle questionnaires.
Missing appointments without prior notice may lead to removal from the programme and cancellation of your membership.
After completing the programme, you will have the opportunity to join one of our memberships to aid progression. The fee is reviewed annually and may be subject to change.
Membership cannot be frozen or suspended during the 12-week programme except in exceptional circumstances.
Programme activities are exclusively available to the registered member.
Before using the gym, you must attend an ‘Activation’ appointment and sign a Health Commitment Statement. You must inform us of any changes to your health or medical history, that could impact your ability to exercise safely.
Everybody Health & Leisure accepts no liability for death, personal injury and loss or any loss or damage, except where such death or injury results from negligence by Everybody Health & Leisure or its employees.
Everybody Health & Leisure may refuse any membership application or renewal at its discretion, without obligation to provide a reason.
You must present your membership card at each visit. The card and membership are non-transferable and remain the property of Everybody Health & Leisure.
Please keep your membership card safe. A £5.25 administration fee applies for a replacement card if lost.
Everybody Members must follow the Customer Etiquette Guidelines displayed in our facilities. Management reserve the right to deny entry to anyone deemed to be acting in a dangerous or inappropriate manner.
A photograph will be taken when your membership card is issued for security purposes. This image is used to verify your identify before granting access to our facilities.
Everybody Health & Leisure may amend or withdraw any of the above conditions or activities, or suspend the programme, in whole or in part at any time without prior notice.
Any personal information you provide will be stored in accordance with the General Data Protection Regulation (EU) 2016/679. It will be used to process your programme membership and where relevant lifestyle data will help evaluate the improve the programme.
